The initial evaluation process includes preliminary information-gathering & review, three hours of clinical interaction, and treatment planning.

part one:

ACCEPTANCE INTO PRACTICE & PAPERWORK

Introductory communications provide an opportunity to clarify any questions about practice philosophies, procedures, or policies. Initial paperwork guides Dr. Trettin to confirm required level of care and initiate basic information gathering. Acceptance into the practice includes reviewing website to understand practice details, completing the eight patient forms, emailing with Dr. Trettin to confirm the relationship is a good fit, receiving/paying the
$200 administrative fee, and obtaining the password to schedule through website calendar link.

PART TWO:

INITIAL EVALUATION APPOINTMENTS

There are three required evaluation appointments. The three required hours can be scheduled in any desired increments of one-hour blocks (one-hour session x3, three-hour session x1, or two-hour session plus one-hour session). These appointments are billed at $400/hour.

PART THREE:

COMPREHENSIVE TREATMENT CARE PLAN

An individualized and comprehensive treatment care plan is shared in the form of a lengthy summary of diagnostic thoughts and suggested integrated medication-therapy goals. Dr. Trettin spends extensive time processing each person’s story and considering developmental, relational, and biological factors. This document requires a $200 paperwork fee and is provided once all payments are up to date.

TOTAL INVESTMENT = $1,600

It is important for people to be able to freely share their story and priorities for care before the thorough evaluation. Following this, disussion shifts to an extremely detailed overview of psychiatric symptoms, medical history, and many overlapping factors including development and current challenges. Using a team-based approach, the above assessment is taken together with individual values to form the begining of an action-based and optomistic plan. This process generally takes three evaluation appointments, each billed at $400/hour.

PART THREE:

COMPREHENSIVE TREATMENT CARE PLAN

An individualized and comprehensive treatment care plan is shared in the form of a lengthy summary of diagnostic thoughts and suggested integrated medication-therapy goals. Dr. Trettin spends extensive time processing each person’s story and considering developmental, relational, and biological factors. This document requires a $200 paperwork fee and is provided once all payments are up to date. Moving forward, this summary is continuously updated so as to maintain clarity and a successful outcome.

Ongoing sessions are chosen based on a combination of patient preference and clinical appropriateness. Choice Psychiatric Healthcare offers three types of appointments:

MEDICATION THERAPY (EXTENDED)

55 minutes

Appropriate for:

• Lengthy conversation about psychopharmacology plan

• Further depth and involvement in psychotherapy process

• Additional time to include partner or family member(s) in conversation

Billed at $400/session

MEDICATION

THERAPY

40 minutes

Appropriate for:

• Most individuals

• Shifting medication regimen or complicated medical issues

• No additional/separate therapist

• Recently established care with Dr. Trettin

Billed at $350/session

MEDICATION

FOCUSED

25 minutes

Appropriate for:

• Long history of care with Dr. Trettin

• Stable medication regimen

• Involvement of additional/separate therapist

Billed at $300/session

PAPERWORK & ADDITIONAL FEES

Appropriate for:

• Complicated medication dialogues

• Insurance or prior authorization issues

• Disability or other forms

• Extended phone conversations

Billed at $200 if time outside of

appointments exceeds 15 minutes
